2012 SCHOLARSHIP APPLICATION
Applicant Guidelines:
- Applicant must be a high school senior, bound for college or an institution of advanced education
- Applicant must be currently employed by a MACC member in good standing
- Applicant must have a minimum GPA of 2.5
- Award will be split equally among qualified applicants
- Check will be payable to the Recipient AND to the Institution of his/her choice

2. A letter of reference from your high school counselor, including transcripts
